Volume Two: The Integrated Personality Treatment Framework: A Clinical Treatment Manual and Client Workbook for Personality Disorder Recovery

If the textbook answers the question of why, the treatment manual answers the question of how. This second volume is a comprehensive, dual-audience clinical resource designed for qualified mental health professionals and the individuals they treat. It is, in the most literal sense, the working clinician's guide to putting the IPTF into practice - structured across thirty-four chapters and seven parts, providing session-by-session protocols, therapist scripts, group facilitation guides, formulation tools, reproducible worksheets, implementation guidance, and a complete client workbook.

The IPTF is organised into three sequential stages, each addressing a distinct level of personality dysfunction and each drawing primarily upon a different evidence-based therapeutic tradition, sequenced according to a coherent clinical rationale grounded in developmental neuroscience and attachment theory.

Stage 1 - Stabilisation and Safety draws upon Dialectical Behaviour Therapy to address the neurobiological level of dysfunction: the emotional dysregulation, impulsive behaviour, and crisis-driven self-harm that constitute the most immediately dangerous manifestations of personality disorder. Across six to twelve months of structured group skills work and individual therapy, Stage 1 equips the patient with a functional repertoire of emotion regulation, distress tolerance, interpersonal effectiveness, and mindfulness skills, establishing the behavioural safety and emotional stabilisation without which deeper exploratory work cannot safely proceed.

Stage 2 - Cognitive Mapping and Schema Work draws upon Schema-Focused Therapy and Cognitive Behavioural Therapy for Personality Disorder, delivered within a mentalising therapeutic stance drawn from Mentalisation-Based Treatment, to address the cognitive and relational level of dysfunction: the maladaptive core beliefs, dysfunctional coping modes, and distorted internal working models that sustain the personality disorder across the lifespan. Through four sequential clinical phases encompassing developmental history exploration, schema identification and mapping, imagery rescripting, chair work, cognitive restructuring, and behavioural experiments, Stage 2 enables the patient to understand the developmental origins of their difficulties and to begin modifying the deep-seated patterns that generate the crises addressed in Stage 1. Typical duration spans twelve to eighteen months.

Stage 3 - Functional Recovery and Identity Consolidation addresses the social and developmental level of dysfunction: the vocational difficulties, social isolation, identity confusion, and absence of meaningful purpose that so frequently persist even after psychological stabilisation has been achieved. Drawing upon vocational rehabilitation, social prescribing, values-based work, and occupational approaches, alongside the psychological modalities of earlier stages, Stage 3 supports the patient in translating internal psychological change into lasting improvement in everyday functioning, culminating in a planned therapeutic ending underpinned by a comprehensive relapse prevention plan. Typical duration spans six to twelve months.

The framework is governed throughout by five active ingredients, structure and consistency, validation and empathy, the therapeutic alliance, affect regulation and distress tolerance, and the cultivation of agency and self-efficacy - identified through common factors research as the mechanisms of change shared across all effective personality disorder treatments. These ingredients operate across all three stages, providing theoretical coherence and clinical consistency to what might otherwise appear as a series of discrete modality shifts.
