Delivering high-quality, safe care requires strategies that follow patients from the hospital to the home. Real-world implementation of quality and safety initiatives must bridge the gap between hospital care settings and home-based services, ensuring continuity, clear communication, and accountability. From standardized discharge planning and medication reconciliation to home health monitoring and caregiver education, healthcare systems must align clinical best practices with practical, community-based support. By integrating hospital teams, primary care providers, home health agencies, and families into a unified care model, organizations may promote improved healthcare outcomes. Real-World Strategies for Implementing Quality and Safety From Hospital to Home Care explores how healthcare organizations apply practical, evidence-based strategies to maintain quality and patient safety as individuals transition from hospital settings to home care. It examines the systems, communication processes, care coordination models, and accountability measures that help prevent errors, reduce readmissions, and ensure safety. This book covers topics such as business strategy, risk management, and workplace culture, and is a useful resource for business owners, medical and healthcare professionals, academicians, researchers, and scientists.

Dr. Badia Faddoul , DNP, RN, CPHQ is the Executive Director of Quality and Safety at Johns Hopkins Care at Home in Baltimore, Maryland. For more than three decades, she has worked across hospital, ambulatory, and home based settings, advancing patient safety, workforce safety, quality improvement, and regulatory readiness. Dr. Faddoul is known for translating complex systems and standards into meaningful practice at the point of care. Her work blends analytic rigor with close attention to the human conditions under which care is delivered, with a particular focus on creating environments where accountability, learning, and psychological safety can coexist. She has led multidisciplinary initiatives spanning patient safety, workforce protection, infection prevention, transitions of care, and organizational resilience. Her leadership philosophy is grounded in presence, showing up where care happens, listening to those delivering and supporting care, and translating what is observed into sustainable improvement. The perspective reflected in this book draws from her lived experience as a nurse, quality and safety leader, and mentor, and from a core belief that quality begins with connection.
